Great idea, filling mine out now. I have always worn a blood type (O, 0-, B, B+ etc) patch on my leather, not only because it is functional but it sparks conversation.

I have found a program that will let you make a very nice medical information (emergency) card. The program is safe to install and no information is shared with anyone. The ZIP file is in my Google Cloud and can be accessed here:

Just download the setup file, install, make your card and if you wish, uninstall right afterwards.

I also have the ICE numbers in my phone, but if it's locked they will be no good. I have put stickers on all my helmets that I got on Amazon the say, "CAUTION: If rider is injured, DO NOT remove helmet"
